Initial Consultation consists of diagnosis of any problems and/or concerns that you may have.
Please bring the following with you to your appointment:
- Your referral slip and any X-rays, CT scans, MRI films
- A list of your current medications, including dosages.
- If you have medical insurance, bring the necessary completed forms and your insurance card.
- Patients under 18 must have parent/guardian
